The family plan will provide coverage for you, your spouse or domestic partner, and all your dependents. Your spouse or domestic partner will receive 50% of the amount listed above. Each dependent child will receive 25%.

No. There's no termination age for this hospital accident insurance. However, benefits reduce due to age, beginning at age 65.

The family plan, if selected, will automatically cover you, your spouse or domestic partner, and all your dependent children. Your daily hospital, ICU and ER benefits will be the full amount you've selected on your enrollment form. Your spouse or domestic partner will receive 50% of confinement benefits and each of your dependent children will receive 25%.
