Medical Coverage

Short-Term Disability Benefits

• Coverage provided by the bank
• Up to twelve-week benefit duration after a seven calendar day elimination period
• Pays 100 percent of salary for the first four weeks and 60 percent for the remaining eight weeks

Long-Term Disability Benefits

• Bank-paid Base benefit equal to 40% of monthly earnings
• Voluntary Associate Buy-up of additional 26.7%, for a total benefit equal to 66.7%
• Guaranteed approval for both base and buy-up (subject to plan's pre-existing clause)
• Benefits begin after 90 consecutive days of disability (coordinates with STD)

Long-Term Care Insurance

• Coverage available to associates and family members
• Plan covers costs of nursing homes, assisted living facilities and home health care services
• Choose from multiple plan designs
• Premiums based on group rates
• Coverage is portable
